If you’re looking into private pay home care (care not covered by Medicare and Medicaid), you may be confused about what services are available—not to mention what sort of care you need.
When you contact Supreme Home Health Services, our knowledgeable patient service personnel will ask you questions to help you figure out exactly what you need. We’ll explain the services available to you, determine how much the services you need will cost, and when you are ready, we can arrange for your care over the telephone—often within 24 hours.
A Registered Nurse will conduct an in-home Initial Assessment to create a customized Plan of Care–a description of all the patient’s issues and needs–for the home health aide to follow. The nurse will visit the patient routinely every 60 days, with additional visits as needed, to review the Plan of Care, make any necessary changes, coordinate all services provided by home care staff, and oversee the care and the staff to ensure that your needs are always fully met.
In addition, Supreme Home Health Services creates a Patient Profile before assigning a home health aide, and our Compatibility Guarantee means that if you’re not happy with your Certified Home Health Aide or Companion for any reason, we’ll provide a replacement.

Round-the-Clock Care
When you need care at home, Supreme Home Health Services can provide Certified Home Health Aides who can ensure the safety of your loved one—and peace of mind for you—24 hours a day.
Check-In by a Nurse
Supreme Home Health Services professionals work around your schedule to meet your needs, whether you would like to receive care for a few days or for several months. In addition to assessment and care planning, our Registered Nurses are able to provide Medication Pre-Pour Service.
Daytime Home Health Aide
If you’re not home during the day, Certified Home Health Aides can provide your loved one with companionship or assistance with activities of daily living, or help getting to and from appointments and outings. Shifts start at a minimum of two hours a day. Aides can:
Assist with bathing and dressing, meal preparation, using the toilet, errands, light housekeeping, and eating
Offer reminders to take medications
Assist with medical equipment
Provide companionship, such as sharing afternoon tea, helping with crossword puzzles and accompanying outside of the home
Respite Care
Primary caregivers are often forgotten in the home care equation. You may be reluctant to run errands, meet a friend for lunch, or take a weekend to attend your niece’s wedding or simply to recharge because you don’t have a trustworthy, knowledgeable caregiver to fill your shoes. What types of services does Supreme Home Health Services provide?
There are a variety of services that Supreme can provide for you or your loved one:

Is there a minimum amount of service?
There is a two-hour minimum per day. Care can be provided from one to seven days per week, depending on the patient’s specific needs.
What steps are involved in implementing services?
The process is very easy. For service simply call 201-372-9600 ext. 33. Working closely with the patient, family and physician, a Supreme Home Health Services registered nurse will recommend a Plan of Care. This plan is tailored to meet the patient’s specific needs. Because no two patients are alike, we are careful to match our caregivers with the personalities and needs of the patient. A registered nurse will supervise your care.
Why is there a two-hour minimum, if I only need a home health aide for one hour?
For home health aide services, we require a two-hour minimum in order to ensure the best possible care and to maintain consistency with the caregiver who is working with the patient.
How far in advance must I call to set up services?
We work to handle all service requests as they are needed, from the same day to several weeks in advance. It is helpful if we are given at least 24-hours advanced notice of the time the service is needed.
Once I call and provide all the necessary information to start services, what is the next step?
Once your call is completed, a home health aide supervisor will be assigned to personally assist you with identifying the right staff member to meet your needs. The supervisor will then contact you to provide the name of the home health aide and answer any questions you may have.
If I need to get in touch with someone at Supreme Home Health Services over the weekend, on a holiday, or during the night, will someone be available to help me?
Yes, someone is in our office five days a week during business hours and we are accessible 24 hours a day. We have staff members who are on-call during all non-office hours to assist you with any urgent service issues.
What languages do the aides speak?
Our aides speak a variety of languages, including Spanish, Russian, Korean, Polish, and Chinese. We always do our best to locate an aide who speaks the patient’s preferred language.
Are Supreme Home Health Services Certified Home Health Aides and Companions screened, and in what manner?
All candidates are carefully screened in the following areas:
Criminal Background Check: Background checks are completed to screen for felonies, misdemeanors, violations, outstanding warrants, and criminal time served. Supreme Home Health Services looks as far back into an individual’s history as possible.
Social Security Check: This check include all counties in which the candidate has lived or worked.
Personal References, Work References
Employee Health Check: Candidates must undergo physical examination and immunization update prior to start of employment.
Proof of Address, I - 9 Verification
Certificate Verification: All Certified Home Health Aides have their certificates verified.
Interview, skills assessment, and entry-level tests, including reading and writing.
A candidate must pass all of the above criteria as well as complete our rigorous orientation program.
