Personal Care and Companionship
Supreme Home Health Services Certified Home Health Aides and Companions respect each patient’s individual needs and desire to maintain quality of life, whether that means regular walks in the park, afternoon tea, reading the newspaper, or discussing current events.
Our aides are assigned for at least two hours a day, with no minimum number of days, and are available to fit into your schedule. Service can be short- or long-term, periodic or continuous. If an aide is not available due to illness or vacation, a replacement aide is always offered to make sure the patient does not have to do without the assistance he or she requires.
In addition, a Supreme Home Health Services Registered Nurse supervises the aide, as well as oversees the Plan of Care and updates it as necessary.
Our Certified Home Health Aides can help with:
- Reminders to take medications
- Recording of vital signs, including temperature, pulse, respiration, and monitoring of pain
- Food shopping, cooking and meal preparation
- Bathing, dressing and grooming (bath service is also available separately as a two-hour visit)
- Short-term personal assistance after injuries or procedures
- Long-term care for people with Alzheimer’s disease and other debilitating conditions
Nursing
Supreme Home Health Services provides highly skilled and experienced Registered Nurses to supervise patient care services.
Our nursing care includes:
- Thorough assessment and individualized care planning, at the time of admission, every six months, and any time patient status or care needs change.
- Monitoring visits every 60 days and as needed
- Medication Pre-Pour Service
Please note that there is no charge for assessments, care planning or monitoring visits. Medication Pre-Pour Services are private pay.
